About this program

Minnesota Department of Health (MDH) is offering multiple open grants under its Rural Health Transformation Program and other initiatives—funded in full by CMS/HHS—to support projects such as a statewide tele-buprenorphine access line, children’s mental health services, healthcare worker retention, Project ECHO for maternal and behavioral health, workplace safety for small rural providers, source water protection, and efforts to prevent sleep-related infant deaths. These opportunities provide program-specific awards with application deadlines ranging in May–June 2026 and require applicants to follow the RFPs posted on the MDH grants site. Eligible applicants vary by program but commonly include Minnesota-licensed hospitals and health systems, FQHCs, rural health clinics, tribal and urban Indian health programs, nonprofit community and behavioral health providers, counties/consortia, and community organizations partnering with licensed prescribers. Applicants must meet the location and program-specific criteria (e.g., rural service area, MHCP provider enrollment, iECHO training, or demonstrated subject-matter experience) and designate a fiscal lead for partnership applications.
