Community Health Matching Grant Program

About this program

The Blue Cross Blue Shield of Michigan Foundation’s Community Health Matching Grant Program supports Michigan nonprofits implementing community-informed or evidence-based initiatives that address identified health needs and measure programmatic and health outcomes. Awards provide up to $25,000 annually for two years, with typical awards of $15,000 per year. Eligible applicants are Michigan-based nonprofit community organizations, which must provide a match determined by organizational budget and the proportion of Medicaid or uninsured patients served; applicants must submit a concept paper before being invited to apply. Projects should address health outcomes and non-medical drivers of health and include evaluation, measurable outcomes, sustainability, and replication plans.
