Financial Assistance for Transportation and Housing During Cancer Treatment

Eligible applicants must reside in one of the foundation’s specified Nebraska counties, be under an active physician-directed treatment regimen at the time of application, have household income at or below 300% of the Federal Poverty Level, and not have received Heartland assistance within the past year. Required documentation includes proof of income (recent tax return, W-2s, pay stubs, or Medicaid verification), mileage to treatment, and physician/facility contact information, and applicants must accept the program’s grant restrictions.
