About this program

The Samuels Foundation Healthy Aging Program provides grants to support patient-based and social service activities that directly serve older adults in New York City's five boroughs, with priority given to high-quality providers that increase access to care, improve quality of life, and serve traditionally underserved populations such as immigrants and economically disadvantaged seniors. Eligible applicants must be tax-exempt, publicly supported organizations providing direct services in NYC that can demonstrate measurable outcomes, cost-effectiveness, and a pathway to sustainability beyond the grant period through revenue generation, reimbursement mechanisms, or other funding sources. Prospective applicants should contact a Healthy Aging Program Officer to discuss their proposed project before submitting a Letter of Intent through the Foundation's online portal. The Foundation does not fund individuals, general operating support, biomedical research, endowments, or capital projects unless directly necessary for program implementation.
