Integrated Behavioral Health Implementation Grant

About this program

The Rapides Foundation offers the Integrated Behavioral Health (IBH) Implementation Grant to support Federally Qualified Health Centers (FQHCs) in enhancing behavioral health services and patient outcomes, particularly for those with comorbid physical health conditions. This grant provides up to $100,000 annually for up to three years to address opportunities identified during the Assessment and Planning Grant process. Eligibility is limited to FQHCs that have completed the Assessment and Planning Grant and are located within The Rapides Foundation’s nine-parish service area in Louisiana. Applicants must be grant-supported FQHCs meeting Medicare and Medicaid criteria and receiving funds under Section 330 of the Public Health Service Act.
