PKD Foundation Community Engagement Grant (Centers of Excellence)

About this program

The PKD Foundation’s Centers of Excellence Community Engagement (Care Team) Grant provides up to $6,000 in one‑year funding, contingent on PKD Center of Excellence designation, to support patient‑focused community programs that advance education, awareness, care coordination, and research‑informed navigation for people with polycystic kidney disease. Fundable activities include patient education seminars, initiatives to reduce underdiagnosis and improve referrals, outreach to underrepresented ADPKD communities, training in cultural competency, support for transplant/dialysis decision‑making, and other measurable, scalable pilot or ongoing projects addressing local barriers and social determinants of health. Applicants must be team members of a PKD Foundation Center of Excellence and include at least two investigators — a clinician or care team member (e.g., clinician, NP, patient navigator) and a patient stakeholder — with involvement of a Patient Advisory Panel strongly encouraged. Grants exclude indirect costs, institutional salaries for project staff, and capital expenses, require final progress and financial reports, proper PKD Foundation attribution for outputs, and are awarded following COE Advisory Panel review.
