VNA Foundation Funding Guidelines

About this program

The VNA Foundation offers grants to support nonprofit organizations delivering health services to underserved communities in Cook County and nearby areas. These grants, typically $40,000 or more, aim to benefit medically underserved, uninsured, and Medicaid recipient populations.

To be eligible, organizations must provide home- and community-based health services, hold 501(c)(3) tax-exempt status, and be designated as a public charity under section 509(a)(1) or 509(a)(2). Applications are reviewed four times a year, and a Letter of Intent is required before the full proposal.
